ERI PLUS - Put Your Ticket to Work for You!
ERI PLUS is an Employment Network under the Ticket to Work Program that supports workers with disabilities to maintain long term employment.
If you are an individual with a disability and have found successful employment on your own or with the help of the Division of Vocational Rehabilitation, we congratulate you! Along with that, we understand you may be facing some uncertainties — if your case with DVR is about to be (or already is) closed or your Social Security benefits are about to end.
As you continue in your job, who can you rely on to help with employment or public benefit issues? Could you use extra money to pay for things that will help you continue working? ERI Plus can help you by providing:
- benefits counseling,
- cash payments to help support employment-related costs, and
- connections to local services.
Put your Ticket to work for you by assigning it to ERI PLUS. You will get continued employment support, expert benefits information, and payments to buy things to help you continue working.
ERI PLUS offers choices to ticket holders:
- TicketShare - a direct cash payment to you (up to 75% of the monthly ticket
payment) to support work related expenses such as car payments, childcare,
bus-fare, clothing, technology, etc.
- Benefits counseling to keep you informed about changing benefit and health
insurance issues.
- Connections to work related service providers across Wisconsin.
Our Mission (or, Why We’re Doing This):
ERI PLUS assists people across Wisconsin receiving Social Security disability benefits to maintain employment, fully understand their individual public benefit situation, and work toward independence.
Potential income that may be generated through the Ticket to Work Program is used to fund services that enhance community employment opportunities for people with disabilities across Wisconsin.
About the Ticket to Work Program
- Ticket to Work is a program developed by the Social Security Administration (SSA) as a support structure for SSA recipients to seek and maintain employment.
- Ticket to Work is designed for SSA recipients who have a goal of becoming financially independent.
- Approved employment support providers (called Employment Networks or ENs) receive monthly payments from SSA for assisting beneficiaries in increasing work earnings. These periodic help to cover the costs of employment services.

General:

Release Forms:
- BPQY Release Forms: These form will help us to verify your recent earnings and your benefit information with SSA. BPQY 1 | BPQY 2
- Employment Information Release Form: This form will help us to verify your wages and employment status with your employer in the event that further confirmation is required.
- Other Release Forms: We may have ask for additional benefits release forms for you to sign so that we can verify other benefits such as Medicaid and SSI State Supplement.
Other Forms:
- Timely Progress Requirements Worksheet: Maximus will conduct annual timely progress reviews (TPRs) to ensure that all Ticket to Work participants are making timely progress towards employment goals. Timely progress simply means that you are making consistent gains toward financial independence while participating in a Ticket program. As long as timely progress is made, you will not be subject to a medical continuing disability review (CDR).
